AxiaLIF®

The Least Invasive Solution to Lumbar Fusion

TranS1 has pioneered a revolutionary approach to access the lumbar spine. The AxiaLIF (Axial Lumbar Interbody Fusion) System includes surgical instruments for creating a safe and reproducible pre-sacral access route to the L5-S1 vertebral bodies. The AxiaLIF technique features novel instrumentation to enable standard of care fusion principles, distraction and stabilization of the anterior lumbar column while mitigating the soft tissue trauma associated with traditional lumbar fusion through open surgical incisions.

Using AxiaLIF, the lumbar spine is accessed through a minimally invasive opening adjacent to the sacral bone. This atraumatic tissue plane alleviates the need for the surgeon to cut through soft tissues like muscles and ligaments, thus lessening patient pain and the likelihood of complications. The TranS1 minimally invasive approach typically allows patients to be discharged from the hospital the day after surgery, and on average return to work in 15 days. This is a dramatic reduction in hospital lengths of stay, which can otherwise run three to four nights, followed by a one- to two-month recovery period before returning to work.

Technique

Access

Make an incision lateral to the tip of the coccyx. Advance the dissecting tool to the docking point on Sacrum.

Establish Working Channel

Use a series of dilators in sacrum. With last dilator, leave cannula docked in the sacrum. Then, drill through sacrum to L5/S1 disk space.

Discectomy and Bone Grafting

Use radial cutters and tissue extractors to remove disc. Insert bone graft into disc space.

Measurement

Measure for the Axial 3D Rod length. Perform an exchange sequence in order to insert the tubular retractor.

Implant Delivery

Select and insert AxiaLIF 3D Rod with Rod Driver.

Closure

Flush and aspirate with anti-biotic as needed. Close the skin in routine fashion and apply a dressing to the access site.

The TranS1 3D Axial Rod is a titanium alloy implant that utilizes a differential thread pitch to attain distraction of the disc space during implant insertion.

Instrumentation

The TranS1 AxiaLIF System is a multi-component system including titanium implantable devices as well as instrumentation made of biocompatible materials such as titanium alloy, stainless steel, and nitinol. This system fits conveniently on a mayo stand, and its instruments follow the procedure sequentially from left to right.

System Highlights:

The nitinol cutters are made to debulk the nucleuos pulpulsis and scrap the endplates in order to create a bleeding bed for fusion. Four cutters are provided, that are large and small, as well as lateral and down cutters. These various sizes allow the surgeon to control and direct the radial cutters to best prepare the disc space.

Tissue extractors are used to remove the diseased disc in order to prepare the disc for bone grafting. These tissue extractors are made of stainless steel and allow the surgeon to twist and grab thes disc after it has been debulked by the nitinol cutters.
