AxiaLIF® 360°

360° Stabilization, Fusion with Soft Tissue Preservation

The AxiaLIF Interbody Device, combined with Facet Screws, provides a truly minimally invasive 360-degree fusion at L5-S1.

The AxiaLIF 360° construct is minimally invasive and requires no muscle stripping. This atraumatic access to facet joints leaves minimal scarring, leads to less post-operative pain and reduced blood loss compared to pedicle screws.

For the surgeon, the AxiaLIF 360° system is easy to use, relative to pedicle screw systems. The small number of procedural components fit conveniently on a mayo stand, freeing up valuable operating room space. The entire procedure performed over a wire, ensuring that proper guidance is always maintained.

Technique

For placement of Facet Screws at L5/S1

When using the dissecting tool under fluoroscopic guidance, in the AP view, the goal is to stay within the lateral borders of the L5 and S1 pedicles. On the lateral view, a good initial bony landmark is the midpoint on the inferior process of L5 angling the tip of the dissecting tool lateral, towards the neck of the S1 pedicle.

Facet Targeting

Advance the dissecting tool through the paraspinous musculature until the tip contacts the bony surface of the inferior articular process of L5.

Advance Guide Wire

Continue to advance the Guide Wire through the S1 pedicle and into the sacral body. Once desired trajectory and depth are achieved, determine the associated screw length based on exposed depth markers.

Dilator and Sheath

Insert the 6mm Dilator and Sheath Assembly over the Cannulated Guide Pin and Guide Wire. Advance the Sheath to the facet joint and remove the 6mm Dilator.

Drill

Using the edge of the Sheath as your indicator while using fluoro, advance the Facet Drill to the selected depth. Do not drill past the termination of the guide wire-this will help keep the guide wire fixed in the pedicle.

Tap

Use the corresponding tap and advance to the previously determined depth.

Insert Screw

Place the Facet Screw over the Guide Wire. Do not thread the Screw head beyond the inferior process of L5.

Instrumentation

The TranS1 AxiaLIF 360 System is a multi-component system including titanium implantable devices as well as instrumentation made of biocompatible materials such as titanium alloy, stainless steel, and nitinol. Like the AxiaLIF system, the AxiaLIF 360 system fits conveniently on a mayo stand.

System Highlights:

The 5mm drill and tap are cannulated in order to follow over the guide wire and maintain trajectory. Both the drill and the tap are laser marked to indicate the depth of each length of screw (25mm, 30mm, 35mm, and 40mm) to ensure proper purchase of the screw.

The 6mm dilator and sheath are introduced over the guide wire to the face of the facet. The dilator is removed leaving the sheath in place. The sheath now act as a cannula to pass the remaining instrumentation and ultimately the screw.
